Is there a way in game to keep track of what levels you've done on hard and also done with 4 players?

No unfortunately there isn’t so you have to keep track yourself.

 

Luckily though, this is not hard. At a bare minimum you only have to play the final chapter of each mission for it to count towards either of these requirements so it’s really easy to keep track. If you’re having trouble, just check it off on a piece of paper or your phone’s notepad as you go along.

I'm totally blind, I'm afraid - is there a way to keep track of the severed limbs in this game?

I think there was one in the Trilogy.

Sometimes the game pop ups a message in the left side of the screen about the progression for certain trophies. The trophy related to severed limbs is easy to get if you play a lot of horde games or if you play the campaign with the 4 players configuration (the number of zombie enemies). Also, if you use the shotgun and grenades, its so much easier to sever the limbs.
