Question on difficulty levels

Since the difficulty is changeable throughout the game, if I play the game on hard but change the difficulty to normal for harder sections of the game, will I still be able to get the Charted! - Hard trophy?

Nope. Changing the difficulty, voids all of the difficulty trophies so tough luck I'm afraid - you have to complete the game from start to finish on Hard if you want that trophy.

No, the trophy descriptions literally mention "Without Changing the Difficulty"

You have to play all the chapters on the same difficult to get the trophy. 
here is a guide from YouTube of a guy doing the game on brutal (the toughest difficult) you could use it for help
