I know there are some NPC on several areas that gives us the trophies, by just telling the stories about them, but i can't find few like: Grimstorm Keep, Plague Tower andThe Sanatorium. From what I remember it's a quest in the Blackdagger Ruins, however that area is currently vaulted which is why you might not be able to access the quest.
