Paramedic completion is more than likely missable.

Paramedic completion is the only thing I've got left for the platinum, and after attempting the 2nd and 3rd island multiple times all attempts resulted in being blown up by enemy fire while barely scratching the ambulance (no need to mention the first island with the spaghetti mafia shotguns) . Don't remember it being this painful in the original version, the amount of NPC spawns might be the cause of the difference. Has anyone completed the paramedic missions other than at the start of the game?

I messed up and completed the story before doing the secondaries. If you have the choice, defintely do all the secondaries you can on Portland before you kill Salvatore. I managed to complete the Paramedic side mission on Staunton Island after completing the story, but to do so I needed to avoid driving around the construction site and the road near the safehouse where possible. Otherwise the columbians with their AKs would just chew through the ambulance health. So for example if I had pickups down near Kenji's casino, instead of taking the shortest, most efficient route; I would loop around the whole west side of Staunton Island to avoid driving past gang members. Some pickups inevitably spawn near enemy gangs which you can't avoid, but the ambulance has enough health that you can tank the few that require you to drive near them. By the time I'd sorted out my routes for pick ups, my biggest obtacle was completing the side mission before the game crashed, not the ambulance health.

Managed to get it done with this strategy, you still need some luck to either not get patient spawns in that little Cartel area or not get shot as you are picking up patients. What really helped was entering the front side of the hospital by taking a longer route and entering from the side where there is the stadium. You will be wasting some time but will be avoiding automatic weapon fire, other than that you can take quite a lot of pistol hits by the Jamaicans and not even smoke will come out of your engine. Thus your only obstacle remains the possibility of a game crash or getting pulled out by the Jamaicans with the bats. In order to avoid getting pulled out the furthest camera view is the most optimal to see if anyone is approaching your door. Still harder than I remember from the PS2 version but doable.

There are trophies related to each, you need to put out 15 fires (Splish Splash), kill 15 criminals (Vigilante - forgot the name), and beat level 12 Paramedic. Further, 'Furious First responder' you need to put out 20 fires (as many tries as you need), and kill 20 criminals (again as many tries as you want) on each island for 60 total. The game however doesn't track per island only overall.

 

Taxi you just need to do 100 fares for the trophy (I suggest saving after 20 in case the game crashes), and Paramedic is just once on any island - recommended immediately on first for those trophies.

 

These missions are endless, once you meet the criteria/trophy you can just quit/stop.

It’s really easy in Portland just after the first mission - go to the hospital get an ambulance preferably you destroy all doors so it’ll be easier , after that do it till lvl 12 no need for each island 1 is enough . As far firefighter as well do it In each island as early as you open it so in total you’ve to extinguish 60 cars divided by 3 island . For the vigilante I used a police car or the FBI kurama , don’t do it in a RIHNO TANK it’s impossible LOL ?? , in the original I did use a tank but now if you use a tank all the criminals well be in cheetahs banshees stingers so basically fast cars same as the fire truck mission 60 divided by 3 for each island , for the vigilante you need a lot and mean it a lot UZI ammo the best is you collect I think 20 or 30 hidden packages that’s I hope that’s help anyone who’s facing some struggle

Is this achievement required for 100% completion? 

I'm not sure if I can do it in this walkthrough.

Can I do all 60 firefighter missions for example on the second island? I gained the splish splash achievement there and it was not that hard. On the first island it's not possible anymore I think. There are enemies around the hospital area that shoot me down instantly and the car gets on fire before I can get out. I think these are the mafia shotgun guys :D

As it was explained earlier, you have to put out at least 20 fires and kill at least 20 criminals on EACH island

I tested taxi fares to see if the requirement is 100 in "all" saved games or 100 in "a" saved game. I did 10 taxi runs on one save, and 90 on another. This does not pop the trophy, so you need all 100 on a single game save.

He means saving after every 20 fares, and continuing from the latest one. Should be kinda obvious that if you load an older save or a new playthrough your stats will be reset.

Yeah, on the original game as well you need to do everything like that the first time you reach Island 2 and 3 or as soon as you start the game.

It's bad design really, I mean if you're doing the races or any sub mission like that, it shouldn't spawn dangerous enemies.

 

I have no idea why they didn't fix it, or all the other glitches that happen when you progress too far in the story. I mean the Purple Nines mission is famous of that, if you've progressed the story and they're all dead, then of course none spawn during the sub quest...

 

If police can be spawned, or not be spawned depending on what Star Rank you are though, I have no idea why they can't just spawn no dangerous enemies when you're doing Paramedic, Taxi missions etc. That just seems like something that could be so easily fixed.

Bad design, or poor quality assurance either way.

I think we're all looking at it (been suckered in to some degree) through nostalgia goggles. I remember these childhood games as perfect with Uncharted level story, RDR2 level polish, and GTA 4 fun.

 

But these are really old games, at the time this was peak open world - now it's bare-bones content, basic mission design, and gimmicky functions. 

 

I don't blame them for what they were but how poorly they were repackaged and ported now - alongside all the other issues people are experiencing.

I used the Rhino tank to get it done. I will admit that it's not as easy as in the past, but the trick is to not chase the criminals. Instead, you want to figure out where they are heading, and try to cut them off. Just touching them with the tank or shooting is enough to kill them.

 

I did it on Staunton Island, and most of the criminals were driving regular cars and sometimes even stuff like thrash masters.

No it’s not missable. I did the paramedic missions on the first island in the original trilogy but forgot to do so in the ps5 version and ended up having to do it in Shoreside vale since Staunton island columbians near the hospital we’re shooting me up to much but shoreside vale actually was doable and maybe I got lucky that I rarely had to pick anybody up we’re the cartel was and mainly the pick up spawns we’re near the hospital or near the airport. So it’s not missable if you didn’t do it on the first island. You can do it don’t give up or start a new playthrough and drive carefully so you don’t add to potential risk of the ambulance blowing up because you will get shot some by enemies so don’t help them by crashing a lot because you will blow up.

I made a post about this in the GTA III Forum last week, trying to warn people.

 

TL;DR: Make sure you do the Wambulance mission first. The reason being: the further you go through the game, the more angry the gangs get at you, and the more difficult it is to stay alive while driving the Ambulance.

 

Do it at the beginning of the game? Less angry gangs shooting at you.

Do it at the end of the game? Almost impossible. All gangs mad at you and shooting at you.

The trophy name for killing 15 criminals is called Going Rogue. And I think I need to save up on a lot of ammo for both vigilante trophies or unless I could probably possibly wait until I can get me a Rhino tank

Not really, I personally would recommend doing it in Portalnd as soon as you can get your hand on an Uzi (20 packages or some Mafia missions) and without Mafia/Triad being hostile.

 

I know I went into is with around 1300 rounds of Uzi ammo and got 15 kills without reaching 750. The criminals will drive and crash themselves 2 clips (50 rounds) is more than enough to blow up the cars. Also if they get damages or stuck they will get out and try to run away on foot where you can easily run them over or get them with a couple of rounds in a drive-by.

 

In short, no need to to hoard massive amounts of ammo.

I feel like they increased the spawns of shotgun-wielding mafia guys. In the original I remember dying a few times to one or two shots to my vehicle but in the definitive edition it took me like 10 minutes to collect one hidden package as I was just getting wrecked the second I drove into the district

In my experience it seems like the Mafia basically forgot I was their sworn enemy.  All the way at the end in Rigged to Blow not a single Mafioso shot at me, as was the case in most of my returns to Saint Mark's area.  Even other missions, rampages rather, Yardies, Columbians, or Nines or Jacks were just standing still for me to pop their heads off.  Love this game, glad they at least did something with it, really hope they rebuild it from ground up in whatever the new engine they're using these days.  But I just finished the game again, so there isnt anything that cant be done.  Though, for some reason I had to start a new game just to get Wreckless Driving to pop.

I already got going Rogue, I hope the Colombians and yardies don't mess with me as soon as I get in Staunton Island soon
