Can’t get the challenge mode trophy to pop

I’ve run through this game twice now on challenge mode and beaten it without losing a match and neither attempt has unlocked the trophy. Anyone know if there is any known issue with this?

 

is it maybe because I need to do it all in one sitting without saving in between battles? Is it because on both runs I did all the quests and all the challenges - do I only have to do story missions to get this to pop? Would love to start the third attempt at this trophy knowing exactly what needs to be done or not done as it were.
