Does it matter what difficulty I do permadeath on for the trophy?

Nope, you can play through on a lower difficulty setting for the trophy if you want. 

But, there is a second DLC trophy that requires you to beat the game on grounded.

You can do them both at once or as separate playthroughs.

No, but do it on Grounded in order to save an aditional playthrough. Also and more importantly, if you activate every cheat (one shot kills, infinite farming items, etc) and use all accesibility options like invisible while prone, slow time while aiming, etc. you can make your permadeath grounded playthrough a very easy task to complete, i did it that way and completed the game in 8,5 hours and died just 6 times (losing 36 minutes) so go ahead and do it that way, it's just easier and faster

just doing my playthrough now on permadeath setting....all deaths so far 3 in all have been falling deaths unbelievable haha

 

and now after 52mins of gameplay....i fall off the bridge with abby up high....jeez....harsh
