If each talisman popped up as you did the quest, then you might have had one partially completed (3/4) that one of the 4 pieces from the wolf counted toward. Leaving you with 3/4 again. I would recommend checking each quest giver again just to be sure that you obtained all pieces from each person. The fisherman has been given all rare fish, all of the eyes of witness, sacrificed followers into gold statues, and have done all tasks up to building the mushroom shrine.
