The Forge Lands

Pauldron of the Horsemen LV5 – Muria still gives you this one at the very beginning

Skeletal Axe of Rending (previously PC only) - Construct Hulk boss fight in the Lost Temple, this weapon scales with you.

Maker Scythe LV10 - Ledge above Deathgrip, use hidden tunnels below death grip chest is right across from a Tomb of Death page

Maker Armor set LV10 - The Foundry's hidden chest (by Triple Lindy trophy) by breaking all the pots

Mace Maximus (Possessed Weapon) LV17 - Ghorn Boss in the Scar extra dungeon

Dark Talons (previously PC only) - The Guardian boss drops this one and it scales with you.

Land of the Dead

Chaos Fang LV22 - Gnasher Boss fight in the Guilded Arena, levels with you

Rusanov's Axe (Possessed Weapon) LV18 - Bone Giant in Judicator Tower

Shadow of Death Scythe LV17 - Bonerivan's final chest by Bloodless Enemy

Shadow of Death Armor set LV17 - City of the Dead, 4th floor eastern room break all the skull post in the SE of the room for the chest to appear (credit to Seisokujin)

Van Der Schmash (Possessed Weapon) LV18 - Wailing Host boss fight in the City of the Dead

Lost Light

Angel of Death Scythe LV20 - In the Ivory Citadel after getting the Voidwalker use the portals to break the Corruption Crystals to make this Hidden chest appear.

Angel of Death Armor Set LV20 - In the Ivory Citadel just before the first tower of water to cleanse there is a portal with 4 large pots behind it. Break the pots to make the Hidden chest appear.

Mortis (Possessed Weapon) LV22 - Jamaerah Boss fight in the Ivory Citadel

Fletcher's Crow Hammer (Possessed Weapon) LV21 - Archon Boss fight in Lost Light

Shadow's Edge

Crow Scythe LV22 - Shadow's Edge over-world, requires the Split form near the Serpent Tome behind the locked gate

Crow Armor Set LV22 - Black Stone (Past) in the Western section just before using the key break all the posts as soon as you enter the Past with the Phase Walker.
