Frank's attitude towards Connor needs to be Hostile at the beginning of the scene.

Go back as far as the sex-club scene, and play from there. Deliberately lose the fight against the deviant lady androids, then shoot them in the back while they try to escape. That will really piss Hank off. 

 

If that doesn't do it and you need to go back even further, continuing to chase the deviant and not saving Hank in the chapter "The Nest" will also piss him off. Play from there and then shoot the ladies as above.

 

In The Bridge scene itself, ask Hank a personal question about his son, and reply to all the other questions with the "cold" option. That makes him mad in that moment. 

 

Edit: You may as well wait until you are ready to do the I'LL BE BACK playthrough, as Hank shooting Connor is one of the deaths you'll need in that run.

Yes.

 

Kara can die in Stormy Night. Just stay still when Todd tells you to and he will kill Alice and then her.

 

The first opportunity for Markus to die is during the Freedom march. Do not recruit John (the security guard at CyberLife warehouses) during the Spare Parts chapter and choose to STAND GROUND then SACRIFICE against the police. They'll shoot and kill him.

Hank not Frank. I’ve been way too hesitant to playthrough this again making all these difficult choices myself.

if kara dies in stormy night, the 'on the run' chapter won't happen and one of connors deaths is in that mission

Good point.

Also, if Conner does not go deviant there are a few more opportunities to die in case you missed one.

You can also die with Markus there if you recruted John. You need to attack the police and fail the fight. You will then have the option to shoot yourself or dont. If you dont, John will safe you, but if you shoot yourself, Markus is dead.
