If Persona 5 itself had a trophy set for NG+ and new difficulties beyond Merciless, here are some to list.

True Violence difficulty:
* Enemies deal x2.5 more damage to you

* You deal only x0.5 of your damage to enemies

* Experience gain is x0.3

* Money gained is x0.25

* Critical damage on you and your enemies is quadrupled.

Grim Reaper difficulty (Not for the faint of heart)

* Enemies deal x3.6 more damage to you, which sometimes results in a chance-based (20%-40%) 1-Hit KO

* Enemies have x2.0 more health.

* You deal only x0.4 of your damage to enemies

* Experience gain is x0.2

* Money gained is x0.15

* Critical damage on you and your enemies is increased five times.

Ironman mode

* You only have one save, which auto-saves for every decision you make, including attacks you choose.
* Your decision matters. One single mistake and you cannot revert it back.

Trophies associated:
* Trip to Hell, Once Again (Bronze) - Finish a New Game+ playthrough.

* Punished Becomes the Punisher (Bronze) - Finish a New Game+ playthrough on Merciless difficulty.

* Iron Man, Iron Man, Iron Man (Bronze) - Finish the game in Ironman mode.

* Get Used to It (Bronze) - Finish a New Game+ playthrough on True Violence difficulty.

* The Devil's Club (Silver) - Finish a New Game+ playthrough on Grim Reaper difficulty.

* Death and Taxes (Gold) - Finish the game in Ironman mode in Grim Reaper difficulty.

Kek. That would be the “Half-Iron” mode. It has all the traits of the Ironman mode, but it’s like what you have said. Ironman mode + Grim Reaper in Persona 5 is a brutal and true experience only for the experienced, like XCOM 2. You use the wrong move or commit just a slight mistake, you are totally done. Adding furthermore, you will start from square one (the beginning of the story) if all of your characters permanently die.
