I got it little after when I received the Eridium Fabricator from Typhon DeLeon

You should have completed In the Footsteps of Giants i believe. 

Possibly bugged in co op.

There are 5 Vault Rewards in total but only 4 are needed for the trophy:

1: Earned in the Vault of the Rampager

2: Earned in the Vault of the Graveward

3: Earned in the Great Vault

4: Earned after finding all the Vault Keys for Typhon DeLeon, then receiving the Eridian Fabricator from him as a gift. - The trophy pops here.

5: Earned in the Vault of the Destroyer - Not required for the trophy.

 

If the trophy didn't pop after getting the 4th Vault Reward, your only option is to start a new character or Playthrough.

im trying to unlock this on tvhm right now - as well as my glitched location trophies will update if it can be cleaned up on this run or you need a new character - tvmh is pretty simple to run through esp if youve been playing in mayhem modes.

All I can think is that I left my game open while I sorted tea out the one evening. When I came back I was in a new location and had progressed the story so players must have joined and carried out.

 to confirm I got damn gina after my third vault in tvhm. but despite cleaning up my locations, I still am missing the promethea and all location trophies.
