I'm a little late to the party here but, in 3 I had my whole save file erased and I was only like a few hours away from the plat. Still to pissed to go back and finish it. Lol

It appears really quickly and is easy to miss.

 

Try pushing the rh analogue stick to the right immediately after you melee the first guy and it should work - if it doesn't work press the analogue stick in whatever direction the 2nd enemy is standing.... Though pushing tighttends to work most times. 

 

I always tried to get both enemies lined up one behind the other / in line of sight as it seemed to make it much easier. 

 

Enemies have to be  very close together, and it is finicky to get tight, took me several tries. 

 

If you make a save before trying then you can easy go back to it if it doesn't work right.

 

Half the time I did them they messed up, but with a bit of practice they get easier.
