hi i finished all the missions on insane including the new ones.

and i still don't have the trophy i tried restarting the game and completing another mission on insane

if anyone else know what to do i really would appreciate it.

I see you have played this game for a few months so it is possible that you have completed some missions on the previous 4-skull difficulty that was called "Very Hard" when there were 6 different difficulty levels. They have since then removed the fourth difficulty but it will still show as if those missions have been completed at 4 skulls. Are you sure you have completed everything on the new Insane? I actually had the same problem last night and eventually found the one I had missed replaying on the new Insane mode.

 

Also the mutators won't count anymore if you are trying with those on.

Nope, four skulls (Insane) is enough.
