Issue with the What really happened III trophy

This trophy is for finding all the facts collectibles in the main story. However I've now run through the game twice finding all of them and the trophy isn't unlocking.

Does anyone know whether the list actually includes all facts, or are there any missing?

I just followed a guide and this trophy didn't pop, so frustrating and I don't really want to go back through the whole game again

 

games are annoying when they don't have some kind of chapter select/encounter select options when it has collectibles
