I thought I had them all after one of my playthroughs, started again and the trophy popped when getting the one in the Outskirts right at the beginning Artifact #9: Tess' List Before you shimmy through the broken wall in the apartment, turn around and pick this artifact up off the nightstand in the other room. I didn't use chapter select and I'm not sure how I missed it! All I can suggest is start again and have a checklist, ticking them off as you go along.
