What do all these routes mean?

Do all the different routes (Route A, Route B, etc) mean each individual route is an entirely new playthrough? So there's going to be close to 15 playthroughs for the plat? NEver played this before so I'm confused by the route thing.

No, you can travel back to previous chapters but you can't go forward back to a chapter you were previously at. So the very first route split is at chapter 11. If you're at chapter 15, you can go back to chapter 11 to get the route split, but once you go back, you can't go forward to chapter 15. One playthrough of the first game is 20 chapters long but when you take the route splits into account, you will need to play at 57 chapters to get all the endings. Not sure about the second game though since I haven't played it yet.

after clearing a route you can start a new cycle, but if you're only interested in getting the route trophies dont save after the credits and use the story tree to backtrack to where the splits occur.
