The first ones that come to mind are Item Collector and Monster Slayer in Hero Must Die. Again.  The first is obtaining all items and the second is for beating all enemies.  To get those, you have to accomplish pretty much every other trophy and some of those are very heavily luck based to encounter what you need to get the entry in your monster and item journals.  Both were a pain in the butt for me because I had very bad luck getting some of the events to trigger.

 

(Dis)honorable mentions go to Hunted 10,000 MAD in Omega Quintet and Hardcore Risette Fan in Persona 4 Golden

The description for it is somewhat ambiguous. Winning all modes means:

  • Deathmatch: have the highest score/cash earned when the match ends.
  • Car Jack City: have the highest score/cash earned when the match ends.
  • Mafiya Work: have the highest score/cash earned when the match ends.
  • Race (all 54 tracks): have the fastest time when the match ends.
  • GTA Race (all 54 tracks): have the fastest time when the match ends.
  • Team Deathmatch: be on the team with the highest score when the match ends.
  • Team Car Jack City: be on the team with the highest score when the match ends.
  • Cops ‘n Crooks: be on the team with the highest score when the match ends; win both rounds of the match to ensure that it counts.
  • Turf War: be on the team with the highest score when the match ends.
  • Bomb da Base II: have at least two players in the match pass the mission.
  • Hangman’s NOOSE: have at least two players in the match pass the mission.
  • Deal Breaker: have at least two players in the match pass the mission.

Monolithic Achievement in Danganronpa V3. Hated that trophy with every fibre of my being. Literally spent 10 or more hours of my life on that piece of shit mini-game. I don't think I've ever been so pissed and frustrated over something in a video game before, and I've had plenty of frustrating moments in FromSoft games, some Castlevanias (Rondo of Blood comes to mind), old NES/SNES games, Cuphead, etc. But nothing tops this, literally nothing.

 

No other bronze trophy, no matter how much of a hurdle it was to overcome, has ever compared to this. Yet.

I'm going to go with Thrilling Comeback on 3on3 freestyle, that requires you to get a winning buzzer beater in a 3 vs 3 game. 

 

It's not really a grind type trophy because you could get it on any 3vs3 game, but it has a lot of variables like:

-Your team being behind or drawing

-The 5 minute game must have at most 24 seconds left and your team to be in possession

-The luck of your own character starting the play with the ball, or relying on a teammate passing it to you or getting a rebound/steal.

-Making the shot itself, and making sure that the shot does not go in too early to not be a winner, or take the shot too late and run out of time.

 

Out of the best part of 750 games played en route to getting the platinum, I think I made 3 winning buzzer beaters, the first one being a complete fluke 3 pointer with my centre character who I used almost exclusively for dunks lol

The most annoying ones I’ve dealt with have already been covered by @Copanele.  The only ones I can add are:

 

Heretic from Warhammer, which required you to do all 20 waves of the Chaos Invasion Mission plus the Bonus Wave in a public online Lobby. This means that you can spend 2 - 3 hours doing this only for your net to give out near the end and void the trophy for yourself. It’s a pretty demanding feat and you need good teamwork, too. If you get disconnected and rejoin, everyone but you will pop the trophy. You must stay connected from start to finish.

 

Mission Specialist from Street Fighter x Tekken (Vita), which is awarded for doing all missions in Mission Mode. I don’t have the motivation to go finish this trophy up, as it’s asking a lot for a bronze.

 

Signs of Friendship from Street Fighter x Tekken (Vita), which is awarded for playing against someone in ad-hoc. For those unaware, you must have two vitas and two copies of the game. Then, you have to be connected to the same network and ad-hoc channel and connect for 1 match. The problem is that vita owners are hard to find as is, now finding someone close by to pop a trophy in a subpar game is a miracle. The other option is coughing about $300 for another vita and copy of the game. This is the trophy that baffles many. There are even a couple people who will pay money to others, so that they can pop it for them.

In my case I must say that the God Of War trophy from ni No Kuni II was the worst bronze trophy I had to deal with (and the last for the shiny platinum =D).

Damn with those Skirmishes more difficult than they should be and appearing randomly in the map =\

Also, the Guardian of the Pond trophy and Fishing Master from Persona were suffocatting, these fishing minigames made me dislike so much the act of fishing.

I spent about an hour trying to get both, and it was horrible.

Your other bronzes are quite challenging and/or grindy, just surprised that this trophy with a 17% rarity would make the list. 

 

I had serious problems going for this trophy on the PS3. You can't tilt the PS3 controller the same way you can with the PS4 controller. I basically spent the better part of a hour going through a series of QTEs because Ethan can't get into any collisions or hit anything. Failing the QTE bit where the police car is driving parallel to you will make him hit Ethan's car. Not hard, but very annoying.

 

The Twins trophy also gave me some trouble since you can't fail a single QTE in that either. PS3 controller wasn't doing what I wanted it to, but then again I've had it since late 2012 so it's seen a lot of wear and tear.

Yup, even used some of those github programs and the excel thing to try and get through it, and I eventually did with one of those, but it's just so luck based that that's really what it came down to in the end. I hear the trophy had an even higher requirement for the score in the Japanese release before they patched it. I don't even want to imagine.

I honestly took one look at those programs and didn't even want to try. Once I told myself I am gonna try at least three times per day until I get it, I got it in about 6 days. I was so happy and it was so unexpected at the same time

Think about it: how many games would you be able to plat in those 250 hours.
