PS3 trophy issues

Fully Threaded is unobtainable. I got 5 challenges done and no trophy. No achievers on this site and found forum threads on other sites that says the same thing.

Mr. October is glitched. I didn't get the trophy when entering the postseason after a successful season (52 matches). Tried download my save from the cloud to re-play the last couple of matches but that didn't help.

Perfection in the Work seems glitched as well. Won in second inning (5-0, 5-0) and CPU opponent never reached a base, and no trophy. This was in a postseason match, so maybe not obtainable there. I did try a 1 inning exhibition game as well, and that didn't work either.

 

Game is on patch 01.00. PS4 version is delisted.
