Losing Weapons

As the title says, I have recently lost a weapon from a workbench within a settlement.

Specifically this weapon was Grognak's Axe. I also noticed that I have lost Grognak's costume.

I went through all of my companions and all items in my workbenches but no luck.

Has this happened to anyone else, not necessarily with the same items but maybe just other weapons or apparel?
