Mission Impossible Trophy

What do you need to do to earn this trophy?  I have managed to avoid getting hit by the lasers and the electric fences but still took damage from enemies.  Needless to say, one look at my profile will show that I still have it locked.  Do I have to avoid all damage?  Did I get hit and not notice.  What do I actually need to do to earn it? ;_;
