Victory in chaos

There have been multiple times where I killed 2 people fairly close after each other, but nothing happens. Does it has to be 2 kills in one blow or am I missing something?

Just your standard double kill type trophy, two kills within x amount of seconds, pay attention to the names of your kills is all I can think of.  You may be killing a player and then a phantom hunter, or even two phantom hunters.
