Yumeutsutsu Re:After is a classic Visual Novel game and a very easy and short game in terms of trophies:

• Play all 5 episodes and make a save before first save in each episode, except in the last episode make 2 saves, one before each choice

• Reach the end of each episode, then reload the save and choose the other options

• The goal is to view all endings and choose all dialogue options

• Last step, go to special voices menu and listen to all 9 of them

Maybe you can tell me, is the ps4 version the same game as one or both of the vita versions play asia just released a while back or is it a completely different

It is the PS4 version released for WW, same game as the Asia release.

If you mean in terms of gameplay and story, yes, it is the same.
If you mean in terms of stackable trophies, there are 3 stacks: one for WW (PS4 & Vita), one for Asia (PS4 & Vita), one for Physical Released by Play-Asia (Vita).
