How do you trigger an operative being kidnapped?

My last trophy is A Dish Best Served Cold, which is to complete a revenge mission. I simply cannot get one of these to trigger.

 

In my entire play-through, I had 1 trigger. I found the guy with the kidnapping info (he was cowering in a Clan Kelly area) and was told to hack them for the whereabouts of the operative, yet no prompt showed up.  I eventually stunned them and still no prompt. I exited the game, as I assumed the mission would reset, returned, and was told the mission failed, and my operative was there back on my team.

 

I've spent an hour or so running around Clan Kelly bases, killing them all, and moving on the the next. I'll use an operative 5 or 6 times and switch to another, so I've done this with 5 or 6 operatives. After all of this I've let my game sit idle with my operative floating on a cargo drone, waiting patiently for a kidnapping, but still no luck.

 

Any suggestions or tips? Thanks!

 

UPDATE: I was never able to get this trophy. I tried everything I mentioned above several more times and it simply wouldn't trigger a kidnapping. So I've given up. Definitely keep an eye out for a kidnapping and perhaps make a backup save before you complete it, as in my case they were very rare. Also, I assume if you play the game guns blazing (I took more of a stealth route), you'll have more odds of triggering a kidnapping in your regular play-through.
