Official recognition (all medals) not unlocking

I just unlocked my last medal and yet i did not receive the trophy, just wondering if the requirements might be diffrent than the description says. Is it all medals or all medals and upgrades? Pretty sure it glitched on me but someone might know how they popped it and the requirements, hopefully i dont need to start over. 

 

Ok so it did glitch on me , you only need medals no need to get all upgrades. I had to restart the game and replay a good part of it again to get all medals. Idk what triggered this glitch so be warry when starting this game. Only takes around 5 hours to plat this game thank god for that.
