Trophies not registering

So I have 2 problemns regarding trophies:

1. I sometimes don't get a trophy I should, and do get it when I meet the requirement a second or third time (f. I. having to replay one chapter of the Walking Dead 3 times for the chapter completion trophy to pop)

2. Trophies that pop, but aren't showing in my trophy list. This has been happening primarily this month. First, it was with Injustice Gods among us, for which a couple of trophies popped when I played, but none were added when I looked at the list on ps4. The trophies were there the next day all of a sudden, however. More recently (yesterday), I unlocked 5 or so trophies from Tomb Raider and The Evil Within (they popped). They didn't show up on the list and they still aren't there after a day. Some of them took quite some time and/or effort to complete. 

 

Anyone else having these problemms? Anyone know why this is happpening and how it can be fixed?
