finish every quest is glitched on me?

ive done everything i an find and the trophy hasnt popped. i even finished the last bunker quest.i see a exclamation point on the far left and when i go to that area it is gone and i talk to th 2 people and all the quests are checked off. not sure what im missing? there might of been a few quests i didnt do the secondary objectives and thats all i can think of

edit i fixed it. i retalked to every single person even the speaker, re listened to all the videos and then went in the bunker and replayed the last mission and made sure to kill every single thing on the map before exiting. it then popped. i have no clue why

Same thing happened to me, OP solution is correct, I kept seeing the exclamation point pop up over quest givers, I went to them, spoke, exclamation mark went away, then I did a random quest and it popped right when I ended it. 

Now I'm somehow missing the one for all elite quests :-)
