It looks like a orange/red icon with a sword and arrow under Property (like where it says Beam weapon. Etc). A perfect, and strong example is the MP Nu Gundam's Fin Funnels. It means you need to use that skill 5 times, for safe measure I used it on 5 different enemies and it popped, because I tried once using it 5 times and it didn't pop so I wasn't sure if it needed to be 5 different enemies.
