How often should I do a full PS5 console format?

Performing a factory reset on a PS5 will not degrade the life of the SSD, as long as you have no games installed. modern SSD's are reliable, but have a limited lifetime for writes.

Before resetting the console check if the console and your games are up to date with the latest updates, there is a chance that your save data is corrupted, start a new game on a fresh save and see if the problem solved.

Another solution would be to delete and reinstall the games, they may have been not installed properly.

a) Try not to have too many games installed at the same time, as for the external HD having too many games, I don't think that impacts directly on the console.

b) Make sure your console can breathe, if it's in a tight space, it may overheat and cause these kinds of things.

c) Take your console to be professionally cleaned, it may be very dirty on the inside, also contributing to the overheating.

d) Rebuilding the database might sometimes help, but if you have like 50 games in the console's SSD, free the space. Just leave what you are ACTUALLY playing. You can download them again later.
