Trophy guide question

I'm a little bit confused about the guide and the Fox rank trophy.

Guide says:

Stage 1: Complete the Game and Obtain the Bandana and/or Stealth Camo

 

Here's what's needed to earn all the trophies in this stage:

1. Start a new game on any difficulty (except Very Easy from Metal Gear Solid: Integral¹).

 

 

So I'm finished the game once on normal (bandana) and soon will finish it for the second time (stealth camo)

 

Then the guide says:

Stage 2: Complete the Game and Obtain the Fox or Big Boss Codename

In this second stage, you'll be aiming to complete the game on Hard or Extreme, while respecting the requirements that have been outlined in the table below. Do feel free to load your save data from Stage 1 to start over with the Bandana and/or Stealth Camo, since not using Special Items is not one of the said requirements, and such items are quite useful.

 

 

How to carry the items from normal difficulty? Tried to load up a clean save with bandana, and it didn't let me to choose a different difficulty...

 

Can anyone clear this up?

Got platinum by finishing Japanese version twice.

 

Looks like you can't change the difficulty when loading a Complete Game save file
