Question for those who've finished their Knightmare playthrough

I recently finished my Knightmare playthrough and as I was leaving the boss room I obtained the trophy for beating the game on Knightmare mode, but not for beating the game on Hard mode. I figured I'd wait, since it took a while for the Easy trophy to pop when I beat the game on Normal. I waited until the credits and eventually back to the main menu, but still no Hard mode trophy. Did my game glitch, or was Hard mode not intended to stack? You would think it would given Easy stacked with Normal mode. Did anyone else experience this?

doesn't look like this was ever answered, but, the trophy guide does say that this issue was patched and should be fixed now.

I can confirm that when I've completed Knightmare mode then Hard mode related trophy also popped.

Moreover, In my second game passthrough @Knightmare I've found all of 22 x Crystal Idols, which game's accounting system reported as 25
