Dead by daylight Near-Death Experience glitches???

Is this trophy glitch or what ? I do this more then 25 time

Or dying state and escape?

It’s not bugged, look at the latest achievers. 
 

You must be down once then escape, even if you get hooked once. Consider being grabbed as being down because it still shows your status as being in the dying state.

 

If you haven’t been down once at the end, go at the exit gate and wait the killer to down you so you fall forward where it counts as an escape.

I wonder if this trophy is glitched on the PS4 version as well. Apparently on PS5, only one person can be downed across the whole team! Feels almost impossible of an achievement

It is glitched. It worked fine for years but got broken few months ago for some reason. Trophy counts progress only if one person gets downed in entire trial. I am not sure if that person should be you or can be anyone. Devs said that they are working on a fix.
