Just finished hell with each character on softcore (online) but no trophy. ??
 

UPDATE: Ok, just got it after killing Baal with my Druid again.

I will try this too with every of my character, hope it works.

It was bugged for me too. I got it with 7 characters on SC online. The way I got it to pop: I started doing baal runs with my characters again. Killed him with pally, barb, then with the necromancer I removed 1 part of the full set of.trang ouls and killed baal. It poped

This game just doesnt know when to stop fucking me over

All characters to hell on hardcore completed in single player, no trophy

now i get to restart my singerbarb, javazon and trapsin in hopes of one of them will trigger it

So much time wasted im suppossed to be finishing my lvl98 not replaying finished characters fml
