The odd occasion it means they hacked. However, MOST of time (like me with Motorstorm Apocaypse) it's because I didn't sync my 2nd PS3 until about 10 months after receiving the platinum. Also, with me, if I get a trophy on Game 1, then go on Game 2 and get a trophy, sync it, then when I look at my list on here, it will still show Game 1 at the top, even though it's not the most recent game I got a trophy on.
