Stuck on "Way of the Tempest"

Started the specialization class mission to unlock tempest and I've collected the 3 aparatuses and the 3 spirit essences, but I can't progress any further in the mission.  The quest just tells me to "gather materials for a bottle of smoke".  I've spoken to Kihm (tempest guy) and nothing.  I've also spoken to Sera to pick up some book she's supposed to give me, but nothing.  (I've completed the game).  Any ideas?

It's a requisition quest, fill in the requisition and you should be able to continue by talking to Kihm.

Yeah every specialization class quest ends with a Requisition.

Sera doesn't give you the book, you'll find it in her "home".

The book is in Sera's room on the second floor of the Tavern (if you recruited her), otherwise you have to buy it. Go to a Requisition Table, craft the item and bring it back to the specialization trainer. The Requisition Table is near Cassandra in Skyhold or at any camp in the wilderness.
