I had completed the story, all side missions and many many many collectible items but was still one skill point from complete. I continued to gather collectibles and when I had 100% Braclets, 100% Totems, 96% Daysha Hands, 0% Hunter's Cache and then while searching the map for more to fetch, a new Orange Side Mission was available which rewarded my final point.
