How do you get the "Empty Embrace" trophy?

Anyone know how to get the "Empty Embrace" trophy?

 

Trophy description reads "Open the Locked Door"
Where is this locked door and how do you open it?

 

EDIT : This is an educated guess, but does it have to do with collecting all 6 poppy flowers and opening a door in the hospital, which you play in 1st person view ❓

Yep after you collect 6 poppy flowers you go to the hospital and open the door.

Can't seem to find flower 5. Already got 4, my last one was in 2-3, but the one that should be in 2-11 isn't showing up for some reason. Has it always appeared in 2-11 for everyone else?

Its random. One always spawns on each run. I think it's always before you reach phase 3. My sixth one was on the platform right after the 2nd phase boss.
