Combo Specialist Trophy Glitched

I'm sure I did all the combos including the 14 hit-combo, in one go, I did not save and I'm sure the enemies did not hit a wall or block my attacks. What am I doing wrong?

Keep doing them until it unlocks. Print out a list and cross them out one by one to be extra sure.

When I was doing this, I was 100% sure that I didn't miss a thing, yet it turned out that I actually missed one of the longer gauntlet/grab combos. Then button outputs sometimes start to seem so similar, it was possible that I went "hm, did I already do this one? I think I have" and skipped it.

You can save all you want and gradually knock the combos off the list throughout the game, that was how I did it, before having to visit one of the last bosses and patiently work through the list again.

Yeah I have the list, I will do that. But I turned off the console, does the combos still counts? I heard that I can not quit the game without doing all the combos

Never heard about the "in one sitting" rule. I've done mine over the course of the whole playthrough. All I know that this trophy is a bit finicky with how it registers the combos, and I ended up combing the list one more time to be extra sure, until it popped during the grabs.

I've heard that if I die the last combo does not count towards the trophy. Is it true? It means that I have to do the combo then do another one (Square + Square) for example and then die.

I've done some searching and here are some points to keep in mind: 

• Each hit must connect, If any part of the combo is blocked or deflected then you must re-do it.
• Any combo which results in an enemy death, even with the final blow of a combo, will not register.
• You must kill the enemy you have performed combos on for them to register to your total.
• This trophy will unlock after you defeat the enemy you performed your last required combo on, it will not suddenly pop mid-fight.
• Each combo must not be interrupted by scenery, that is to say that you enter into a mini-game because you were performing the combo too near a wall or the edge of a platform. This includes the final hit which may propel your enemy backwards and initiate the small cutscene of them almost falling.

I hadn't any problem unlocking the trophy following these rules. Honestly I don't think the trophy is bugged at all.

Just wanted to add a piece of advice since I had to do the whole list 2(alsmost 3) times to earn this trophy.

 

Don't forget when you are doing a combo that ends with circle (O) you are not supposed to throw the enemies just to lift them up. Sometimes when combo ends with circle (O) and you tap it too fast it registers twice meaning it will first lift the enemy and then throw him.
