Is there another way to get this trophy easy? 

 

I'm having a big trouble to pass the first monsters encounter.

Just got this trophy and want to share a tip.

 

1. Pass the first checkpoint;
2. Get the Blue Card;
3. Kill the all the Possessed Scientist on the ground after getting the Frag Granade;
4. Kill the 2 Imps that will appear;
5. Wait for the Checkpoint to pop;
6. Try to do the glitch, if fail, exit to main menu;
7. Open slot and CONTINUE game.

 Woh man this was pretty hard.  Long ago I did this first level with a glitch for the trophy, but was able to do it legit now too.

 

First level of Ultra Nightmare without glitches, hope this helps everyone.

Try not to miss two shots, especially consecutively. I'm not sure if this is a detection thing that iD software injected in the code but I noticed (while I was doing this trophy), as soon as I missed x2 shots, the enemy A.I would make it near-impossible to survive the next 5 seconds. Sometimes it would escalate with one missed shot.

I've completed the story mode and Ultra nightmare mode didn't unlock for me, Am I doing something wrong ????

You have to start it in a new save slot.
