This is my full video and text walkthrough for The Medium, either can be used by themselves. Following these will get you all the trophies between 2 and 3 hours and should keep the difficulty to a minimum.

JACK'S APARTMENT

– Find the tie clip –

Go into Jack's room and open the small red box.
Activate insight, then move the clock and get Jack's Tie Clip.

– Get to the Funeral Home –

Go into the kitchen and get the Cat Food from the counter.
Put the Cat Food into the cat bowl.
TROPHY 1/40 - Famished Feline - Feed the cat in Jack's apartment.
Take the door opposite the kitchen door and exit onto the balcony.
Go downstairs and into the doorway left of the blue van.
Open the double door on the right with the Funeral Home Key.

– Prepare Jack for his last journey –

Get the Prep Room Key from on the desk.
Unlock the prep room door with the Prep Room Key.
Go into the morgue and pull out Jack.
Interact with his collar.
Now pick up Jack's Tie from the nearby stall.
Combine Jack's Tie with Jack's Tie Clip.
Interact with Jack and place Jack's Tie with Clip onto his collar.

– Check upstairs –

Go upstairs back into the study and interact with the broken vase on the floor.
TROPHY 2/40 - The Last Goodbye - Send Jack's spirit away.

THE NIWA RESORT

– Get through the gate –

Go into the small office ahead of you, interact with the suitcase inside and take the postcard.
Groundskeeper's Postcards 1/11 - Niwa Says Hello.
TROPHY 3/40 - From Niwa with Love - Find a postcard from the groundskeeper.
Now go outside and around the back of the office, then climb over the wall using the dumpster.

– Make your way deeper into the resort –

Go to the end of the road and climb down.
Head down into the woods to trigger a cutscene with a ball.
Follow the trail through the woods to reach a stone wall with some archways and a doorway.
Go through the doorway and squeeze through the gap between the walls
Cimb the ledge on the left.
Absorb the energy at the end of the tunnel in the spirit-world.
Go back to the fork with the gate and activate the electrical box on the wall with your spirit blast.
TROPHY 4/40 - Conduit - Use Spirit Blast to create electricity.
Now go out through the gate.
Follow the left path to eventually reach a fallen tree blocking the path and interact with it.
Once in the parking lot, go to the far left corner to find a red car and interact with it.
Grab the postcard inside.
Groundskeeper's Postcards 2/11 - Honest Work.
Now flick the switch on the dashboard to release the trunk.
Now open the trunk and get the Screwdriver from inside.
Leave the car and head right over to the payphone.
Interact with the payphone and pick up the handset.
Activate insight, then rotate the handset until you find the echo.
Echo 1/28.
TROPHY 5/40 - Left Behind - Find an Echo.
TROPHY 6/40 - Caught the Scent - Find an Echo of a mysterious caller.

– Get inside the hotel –

TROPHY 7/40 - Welcome to Niwa - Reach the hotel.
Head over to the front door, then go to the door just to the right, interact with it and use the Screwdriver to unlock it.
Get the dumpster inside and push it over to the canopy above the front door.
Then use the dumpster to climb up onto the canopy for a cutscene.

– Search for Thomas –

Activate insight, then pick up the shoe beside you and rotate it to find another Echo.
Echo 2/28.
Grab the postcard sitting on a stall in the far right corner.
Groundskeeper's Postcards 3/11 - Promotion.
Go over to the locked gate for a brief cutscene.
Go interact with the bell on the reception counter for a cutscene.
TROPHY 8/40 - The Cheerful Kind - Meet Sadness.

– Meet Sadness on the second floor –

Head over to the elevator and press the button to open it, then head inside and go to floor 2.
Enter the out of body experience, then go and absorb the energy before going back into your body.
Once back in the elevator, leave your body again and then use the spirit blast to power the electrical box.
Then return to your body.
You are currently on floor 1, press the button for floor 2.
Go into the hallway on the left of the screen for a small cutscene.
Go to the door further down the hallway on the left and then leave your body.
Now go inside the room and grab the Odd Key inside, then return to your body.
Go back down the hallway and use the Odd Key to open the door on the right, then go inside.
Go through the bathroom and into the next room and interact with the telephone beside the window.
Use insight to locate the echo on the telephone handset.
Echo 3/28.
Drop down the hole in the corner.
Go to the square air conditioning unit in the room you fall into and examine the book on it to find a memory shard.
Memory Shard 1/10.
TROPHY 9/40 - Inner Investigator - Inspect 25 objects in a single playthrough.
TROPHY 10/40 - I Know the Pieces Fit - Reconstruct a Memory Shard.
Go outside onto the balcony and go right until the floor breaks away.
Now go out of body and shimmy across to the other side.
Inside the end room, interact with the prompt around the corner from the memory to examine a door hole, then move the cursor around the blood beside the bath to unlock the bathroom door.
Examine the bath and pull out the plug, then pick up the Razor Blade.
TROPHY 11/40 - Edge of Sanity - Acquire the razor.
Now go back to your body.
In the spirit world, interact with the skin on the nearby wall and cut it using the Razor Blade.
Go into the next room and pick up the Dried Up Sunflower.
Interact with the nearby bowl of dirt and unlock the next memory.
Memory Shard 2/10.
Head into the main bathroom, examine the mirror and then use your insight to find another Echo.
Echo 4/28.
Go back into the previous room where you dropped down the hole.
Interact with the vase and place the dried up sunflower inside it.
Now absorb the energy.
Go into the bathroom, interact with the sink and pick up the note.
Mentor's Diaries 1/7 - An Apology.
TROPHY 12/40 - Guiding Light - Find a page from a mentor's diary.
Now interact with the pill bottle, use your insight and get the echo.
Echo 5/28.
Go into the hallway and head forward to the end while using your spirit shield to avoid the moths.
TROPHY 13/40 - Medium Rare - Burn through the moths using Spirit Shield.
Go upstairs, push the laundry trolley out of the way and enter another hallway.
Now head forward and cut the wall of skin using the Razor blade in the spirit world.
Run through the moths while using your spirit shield and enter the room you pass on the left afterwards.
Interact with the box on the floor and then pick up the note inside it.
Troubled Man's Notes 1/20 - A Peaceful Place.
TROPHY 14/20 - A Dangerous Method - Find a note from a troubled man.
Now interact with the nearby door for a cutscene.

– Investigate Thomas's Office –

Interact with the far side of the desk, then pick up the Flashlight.
Also pick up the Small Key that was under the flashlight.
Take the note from the desk drawer.
Troubled Man's Notes 2/20 - To-Do List.
Go over to the clock and use the small key to open it.
Now interact with the clock hands and reverse the time to 04:10.

– Search the hidden room –

Go over to the desk and absorb the energy in the spirit world.
Now interact with the clock again and turn the time forward until it is 04:50.
Go out of body, then go through the opening and activate the electrical box with a spirit blast, then return to your body.
Now open up the cabinet, use your insight and press the button behind the picture.
Go into the secret room.
TROPHY 15/20 - Insightful - Use Insight for at least 15 minutes - Always keep this active until the trophy pops.
Go to the desk on the left, interact with it and grab the Photo Paper.
Now interact with the nearby chair and pick up the Fixer solution.
Also take the Metal Tray inside the square basket.
Take the Multigrade fluid from the shelf.
Interact with the symbol on the floor and take the note beside the center.
Trouble Man's Notes 3/20 - Getting Stronger.
Grab the Stop carton in the sink.
Interact with the workstation and press the switch on the wall. Be quick doing these next few steps otherwise you will ruin the item and will need to repeat it.
Place the Metal Tray in the space on the desk.
Pour the Multigrade into the left tray.
Pour the Stop into the middle tray.
Pour the Fixer into the right tray.
Place the Photo Paper beneath the illuminator and turn it on. Then pick it back up after 5 seconds to get the Illuminated Paper.
Now place the Illuminated Photo into each tray from left to right and keep it in each for 3 seconds.
You will trigger a cutscene once you finish the puzzle.

– Escape the monster –

During the chase sequence, take the path listed below.
1. Right (from the camera perspective).
2. Right.
3. Right.
4. Left.
5. Right.
6. Left.
Once safe, go through the next area WITHOUT alerting the enemy. It's best to crouch as soon as you enter the area and don't worry about the object that falls on the floor and startles the monster briefly, it's meant to happen! Never alert this enemy in any future encounters until the Phantom trophy pops. If you do alert the monster, reload the last checkpoint, but never choose continue.
Take the energy from the safe spot, then activate your spirit shield and run through the moths.
Shimmy across the broken floor at the end for a cutscene.
TROPHY 16/20 - You Saw, You Ran, You Lived - You survived a monstrous encounter.

– Follow Sadness –

Go and interact with the radio on the counter.
Head down to where you initially dropped into the hotel much earlier and go through the double door opposite, then interact with the dayroom door.

– Find a way into the Dayroom –

Go up the nearby steps and pick up the note on the left.
Troubled Man's Notes 4/20 - Fever Dreams.
TROPHY 17/20 - Dual Detective - Inspect 50 objects in a single playthrough.
Head through the doorway you can see on the same level as the note above and follow the path to eventually drop down into the pool area.
Go around the pool, into the shower room and into a corridor.
Then go upstairs into another hallway.
Come towards the camera and pick up the note on the floor to the right.
Troubled Man's Notes 5/20 - Good Old Frank.
Now run to the far end of the hallway and interact with the debris at the end.

– Get the bolt cutters –

Go back a little down the hallway and interact with the mirror on the right.

– Follow the spirit trail –

Open and go through the nearby door, then go right from the camera perspective and pick up the spinning top, rotate it to find another echo.
Echo 6/28.
Now drop down into the empty pool.
Pick up the dog teddy in the middle and rotate it to find another echo.
Echo 7/28.
Climb the ledge where you just dropped in, then pick up the teddy and rotate it for another echo.
Echo 8/28.
Drop back down into the pool and interact with the 'Milicja' sign on the left to find another echo.
Echo 9/28.
Climb out the pool beside the sign and examine the next 'Milicja' sign you pass to find yet another echo.
Echo 10/28.
Go back into the shower room, push the trolley, then pick up the toy tank and rotate it to find a echo.
Echo 11/28.
Go through the crawlspace into the next shower room.
Interact with the green lockers in the corner, open the locker and take the White Cat Figurine.
Now examine the bloody clothes on the floor a bit further along the wall to the right and you should find a echo.
Echo 12/28.
Now interact with the box in the opposite corner of the room and pick up the note inside it.
Groundskeeper's Postcards 4/11 - Someday.
Go back upstairs and interact with the mirror again for a cutscene.
TROPHY 18/42 - Through the Looking Glass - You used a mirror to cross over.
Go upstairs and examine the corpse stuck in the wall at the end.

– Send Bernard's soul away –

Now head the opposite way and follow some blood into a path on the right to find a spirit well, then absorb the energy.
TROPHY 19/42 - Running Sim - You have run 2 kilometers.
Now go back into the corridor and cut the skin on the wall opposite using the razor blade.
While using your spirit shield, go into the opening and pick up the Mask of a Boy stuck onto the central pillar.
Go back to the corpse and use the mask of a boy on it.
Then take the Black Cat Figurine that appears where the corpse was.

– Get the bolt cutters –

Sprint back into the room with the spirit well and interact with the mirror.
Go through the hole in the wall between the lockers and Maw will appear again.
Go to the far end of the room and you will get a prompt to hold your breath. Now hold your breath until Maw breaks down the nearby door. Make sure you don't alert him here from his intended route
Go through the door and into the examination room opposite, then interact with the mirror inside.
Go outside the room and interact with the 2 corpses stuck inside the wall.

– Send away the souls of the other victims –

Go through the path on the left, up the steps and climb up onto the floor above.
Take the Mask of a Man from the wall to the right to reveal a path.
Head through the path into a big room, then turn left and use the mirror at the end.
Pick up the telephone on the table to the left and rotate it to find a echo.
Echo 13/28.
Go into the corridor and enter the far room on the left.
Examine the ashtray on the far table and rotate it to find a echo.
Echo 14/28.
Now interact with the note on the middle table and examine #9 on the list.
Now interact with chair #9 on the left nearest the camera.
Next examine chair #4 opposite #9.
Go back to the note and now examine #4 on the list.
Now make your way back through the mirror while carefully avoiding Maw.
Now go back down to the lower floor and back through the mirror you came through earlier.
Once back in the material world, grab the Examination Room Key beside the bin.
Now pick up the nearby telephone on the wall, then rotate it to find a echo.
Echo 15/28.
Open the nearby door with the examination room key and go inside.
Go left from the camera perspective and interact with the silver bin in the corner to find a note.
Groundskeeper's Postcards 5/11 - A Scenic View.
Now follow the railing around to the opposite end and use the mirror you reach.
Go right and shimmy along the wall to reach another mirror and use it.
Head away and left to find Maw, then crouch beneath the stacked furniture in the far corner.
Squeeze between the 2 shelves on the left to encounter Maw again.
Carefully make your way into the hallway and then squeeze between the furniture at the end.
Go into the nearby room and pick up the ballerina photo from the smaller table, then rotate it to find an echo.
Echo 16/28.
Use the nearby mirror, then enter the hole on the left, go into the room behind the next mirror and take the Mask of a Woman stuck to the wall.
Now go to the last mirror you passed and use it to enter a room that has a bed with a red headboard
Go to the dresser in the corner and pick up the bracelet, then examine the 2 names on it.
Now use your insight and pick up the Torn Notebook Page on the floor at the left corner of the bed.
Pick up the notebook beside the bed and use the torn notebook page on it.
Now examine the 3 middle prompts on the notebook followed by the phone number last which should reveal the name.
Exit the room via the main door to enter a hallway, then use the mirror on the right.
Head away from the camera and shimmy back along the ledge, then use the mirror to warp back to the examination room.
Leave the examination room via the entrance door and use the mirror near the telephone.
Go to the 2 corpses and place both the masks to reveal a path.

– Get the bolt cutters –

Go into the new doorway on the right and use the mirror.
Now go out into the hallway and grab the bolt cutters.
TROPHY 20/42 - Cut the Crap - You acquired the bolt cutters.

– Enter the Dayroom –

Go back into the previous room and through the hole to the left of the mirror.
Examine the wheelchair in this next room to find a echo.
Echo 17/28.
Go out the door into another hallway, then go down and use the bolt cutters on the chained door at the end.
Once you pass through the now unlocked door, as long as you never alerted Maw in any previous encounters, you should get the trophy below.
TROPHY 21/42 - Phantom - Escape the creature without alerting it.
Now go down the stairs and use the bolt cutters on the next chained door you reach.
Go through the door into another hallway, then go up and use the bolt cutters on the chained door on the far left.
Keep following the path until you're back in the lobby outside the dayroom.
Before you go into the dayroom, go back into reception and to the chained gate from earlier, then unlock it with the bolt cutters.
Now grab the note from the table at the end.
Troubled Man's Notes 6/20 - Dust and Death.
Now go back to the dayroom, unlock it and then head inside for a cutscene.

THE DAYROOM

– Investigate the Dayroom –

Head forward and cut through the skin.
Go to the box on the left, then pick up the note along with the horse drawing which has a echo on it.
Mentor's Diaries 2/7 - Throwing Away Greatness.
Echo 18/28.
TROPHY 22/42 - Psychic Sleuth - Interact with 100 objects in a single playthrough.
Pick up the Music Sheet Part 3 from the table on the right.
Pick up the note from the chair on the far right.
Mentor's Diaries 3/7 - A Well of Inspiration.
Go to the closed door in the far right corner and exit your body.
Now go through the doorway and take The Equator which is in front of the camera on the left, then return to your body.
Go left and examine the blood bag on the stand and rotate the stick for a memory shard.
Memory Shard 3/10.
Now take Music Sheet Part 1 from the nearby chair.
Go into the nearby room.
Take the note from the nearby table.
Mentor's Diaries 4/7 - A Method to Madness.
Interact with the globe in the middle, focus on the spirit world and use the equator on the object.
Now interact with the bottom prompt, followed by the bottom prompt again.
Then interact with the bottom prompt yet again, then the middle prompt and finally the bottom prompt once more to complete the puzzle.
Now rotate the globe until it has made a doorway onto the balcony.
Go out onto the balcony and then head down into the next room.
Interact with the piano keys on the corner table and rotate the stick to trigger a memory shard.
Memory Shard 4/10.
Grab the Teacher's Room Key from the table on the right side of the room.
Then open the nearby skin wall with your razor blade.
Go through into the previous room, then go to the couch on the right and take the note from it.
Mentor's Diaries 5/7 - A Very Special Boy.
Go to the door in the far right corner and unlock it with the teacher's room key, then go inside.
Pick up Music Sheet Part 2 from the table in the left corner.
Now combine the 3 music sheets to make the complete Music Sheet.
Get the note from the desk as well.
Mentor's Diaries 6/7 - Growing Apart
Interact with the glasses just to the right of the previous note and rotate the stick to make a memory shard.
Memory Shard 5/10.
Now pick up the pen case on the table in the opposite corner, then rotate it to find a echo.
Echo 19/28.
Go back into the music room, interact with the piano and use the music sheet on it, then absorb the energy.
Now head into the stairwell and head downstairs, use your spirit shield to avoid the moths.
Now go up the small steps to the far right and interact with the painting to get a memory shard.
Memory Shard 6/10.
Go to the table in the nearby right corner near where the stacked tables are and take the note on it.
Mentor's Diaries 7/7 - A New Home.
TROPHY 23/40 - Devouring Darkness - Find all pages from Richard's diary in a single playthrough.
Interact with the doll on the table in front of the stairs for another memory shard.
Memory Shard 7/10.
Now go to the table on the left and examine the ribbon for a memory shard.
Memory Shard 8/10.
Now interact with the wheelchair in the middle for a cutscene.
TROPHY 24/40 - The Childeater - You experienced your first vision of Thomas.

SPIRIT THOMAS

– Free Richard from his demons –

Follow the path ahead which will take you around to the left and into a house. Use the spirit shield to avoid the tentacles along the way.
Take Father's Knife from the table on the left.
Now head through the door that opens behind you and go through the door at the end of the corridor.
Interact with the desk and then examine the pipe on it.
Then use your insight and open the desk drawer, then examine the sketchbook inside.
Now interact with the fireplace, then examine the medal inside the small case on the corner of the fireplace.
Also examine the photo on the right.
Now examine the fire pit and use father's knife on the envelope to find a letter.
Now absorb the energy from the coffin that has appeared on the desk.
Now head over to the large painting on the left side of the room and use a spirit blast to destroy the vines covering it.
Go through the maze and keep right at the first fork, then go left at the next fork to eventually reach a green bench with a cloth and some food on it.
Go right here from the camera perspective followed by another left and keep following the path to reach a gazebo.
Now go back to the lobby and go up the stairs on the left nearest the table that had father's knife from earlier.
Once at the top of the stairs, enter the door ahead.
Interact with the chair on the left.
Now go to the small table to the right and take the Empty Bottle while using insight.
Place the empty bottle on the chair.
Now go and pick up the Wedding Photo and now place that on the chair.
Next, interact with the shelf and take the Book on the left, then place this on the chair, but be ready to use the spirit shield to destroy the tentacle.
Go into the newly revealed room and absorb the energy inside.
Now go into the corridor and into the room at the end, use your spirit shield to deflect the attacks.
Use the spirit blast to destroy the vines covering the painting.
Go forward into the maze and go right at the first fork, then left at the next fork and continue along the path to another gazebo.
Now go back into the corridor and make your way into the room at the end.
Interact with the desk in the middle and examine the 'Collaborator' sign.
Now interact with the furniture just to the left and examine the note.
Next, go to the small cabinet a bit further up on the right and interact with the jar of shinies inside it.
Now interact with the noose to absorb the energy.
Now use spirit blast on the painting on the far wall to destroy the vines.
You will now be back in another maze, go forward and head right at the first fork but be ready to counter any enemy attacks.
Go right at the next fork and keep following the path keeping to the right to eventually reach a rose bush, then interact with it.
Now backtrack to the previous fork and take the 1st path on the right followed by the left path.
Then carry on following the path to the next gazebo and examine the bush inside.
Once back in the house, absorb the energy from the door ahead.
Now continue along the path and use your spirit blast on the tentacles at the end.
TROPHY 25/40 - Unforgiven - You sent the Childeater away.

THE FOREST

– Make your way through the forest –

Go down the path until you reach the cross-junction.
Take the left path, then left again and take the note from the bench.
Groundskeeper's Postcards 6/11 - Won't Stop.
Carry on to the telephone booth, then pick up the receiver and rotate it for a echo.
Echo 20/28.
Go back to the junction and take the top path to reach a clearing.
Grab the note on the bench to the right of the viewing scope.
Groundskeeper's Postcards 7/11 - Poor Thing.
Now go down the nature trail on the right until you reach a fork.
Take the left path and take the note on the first bench you pass.
Groundskeeper's Postcards 8/11 - Harder and Harder.
Carry on forward on the path ignoring the branching path you will shortly see on the right.
Eventually you will reach a broken bridge that you will have to go underneath. You will then have to climb up and will shortly encounter a cutscene.

– Follow the dog –

Ignore the dog for a moment and take the path on the right, keep left though as you are looking for a bench hidden out of view with a note on it that you need.
Groundskeeper's Postcards 9/11 - Whispers in the Dark.
Once you have the note, go back to the fork and follow the dog up the hill until you eventually reach a tent and trigger a cutscene.

– Investigate the camp –

Go to the bench beside the campfire and pick up the note.
Groundskeeper's Postcards 10/11 - A New Companion.
Now interact with the tent and pick up the note beside the boots.
Groundskeeper's Postcards 11/11 - Regrets.
TROPHY 26/40 - All That's Left Unsaid - Find all of Frank's postcards in a single playthrough.
Now pick up the Staff Photo beside the corpse.
Then examine the corpse and exit the tent for a cutscene.

– Escape the monster –

Complete this chase sequence to finish this section.

THE RUINS

– Find a way through the ruins –

Climb up into the ruins and then drop down through the doorway at the top.
Keep dropping down and go across the bridge.
Now cross over the plank on the top right to reach the far side.
Follow the path up and right into the ruins, then go into the next room and push the metal racking so you can access the doorway.
Go through and squeeze through the wall space opposite.
Now take the doorway on the left and follow the steps down into the sewer.
Go forward through the gate, then continue forward for a cutscene.
After the cutscene, sprint away from the monster and go through the crawlspace at the end.
Now quickly interact with the cement mixer and pull it back to magically seal the room.
Use the bolt cutters on the chained gate to the right to unlock it.
Go up the steps and use the bolt cutters on the next gate to unlock it.
Head through this gate to get back outside, then go through the archway at the top left.
Grab the Old Gas Canister.
Go up from here and climb up the platform, then use the bolt cutters to cut the chain and release the 2nd part of the platform.
Climb up again, then turn immediately right and follow the path around and across the 2 wooden bridges.
Avoid the monster here and squeeze through the wall space in the far left corner.
Go over the bridge and climb the ledge.
Now go forward and shimmy along the ledge on the right.
Pick up the Lever from the broken wooden platform.
Carry along the path to reach another closed platform that you can release with your bolt cutters.
Go over to the generator, take off the top cap and use the Old Gas Canister on it.
Now press the button on the generator to start it.

– Open the gate –

Interact with the broken gate lever on the right and use the Lever to repair it and to open the gate.

– Get rid of the monster –

Push the generator until it falls of the track.
TROPHY 27/40 - Thunderstruck - You defeated the creature in the flooded ruins.

– Find a way through the ruins –

Go into the water and through the archway on the far left, then enter the gate you saw earlier.
Go down the stairs to find yourself back in the forest, then continue on for a cutscene.

– Find the Red House –

Examine the bike wheel on the left opposite the house and rotate it to find an echo.
Echo 21/28.
Pick up the cigarettes on the logs to the left of the house and rotate them to find a echo.
Echo 22/28.

– Investigate the toolshed –

Go into the house and examine the horse toy on the worktop to find an echo.
Echo 23/28.
Examine the picture on the shelf above the toy to discover a note.
Troubled Man's Notes 7/20 - Hell's Angel.
Go into the next room and pick up the tape recorder immediately on the left and examine it for an echo.
Echo 24/28.
TROPHY 28/40 - Followed the Trail - Find all of Henry's Echoes in a single playthrough.
Now take the Doll on the table to the right, then exit the house.
Place the doll on the bench to the right of the entrance and then absorb the energy.
Now go out of body.
Quickly climb onto the small roof on the left side of the house and then go out of body.
Climb into the window and then use a spirit blast to power the electrical box.
Go back into your body, then quickly head back into the garage and press the switch on the hanging control box at the back to raise the engine.
Examine the suitcase at the back to find another echo.
Echo 25/28.
Pick up the note on the shelf below the suitcase.
Troubled Man's Notes 8/20 - Something Broken.
Go down into the maintenance hole and pick up the Hydraulic Jack Handle.
Now go over to the garage door and use the hydraulic jack handle on the jack to raise the door.
Now interact with the garage door to crouch beneath it and go outside.

– Investigate the burnt-down house –

Run over to the steps leading up to the house ahead to trigger a fire.
Now the basket to the left of the steps and get the memory shard.
Memory Shard 9/10.
Follow the spirit trail around to the left side of the house and examine the gas canister for another memory shard.
Memory Shard 10/10.
TROPHY 29/40 - Puzzling Out the Past - Reconstruct all Memory Shards in a single playthrough.
Follow the trail back to another object in the grass and interact with it for a cutscene.
TROPHY 30/40 - The Hound - You experienced your second vision of Thomas.

SPIRIT THOMAS 2

– Find a way to stop Henry –

Follow the path and use your spirit force to move the objects.
Follow the linear path upwards, shimmy across a ledge and eventually you will enter a room.
Interact with the monitors in the corner and examine the one on the right.
Now pick up the Reel Tape on the nearby table.
Then use the reel tape on the machine to the right where you will see a free slot.
Now press the play button to begin the recording and open the door on the left.
Follow the linear path, you will need to crouch beneath some pig carcasses and press the button on a hanging control box along the way.
Once you reach the suitcase at the end, open it up and examine the small knife inside.
Now absorb the energy on the left, then go through the crawlspace on the right and continue along the path.
Once you reach the blocked double door, use a spirit blast to clear it, then carry on through and into the small room at the end.
Examine the window until it breaks.
Interact with the table, then examine the ID and the newspaper.
Now go to the desk and take the Reel Tape from the drawer.
Use the reel tape on the nearby machine and then press the play button to play the recording and unlock the door on the left.
Carry on along the linear path and once you reach the creature, quickly pull down the wall to stop it.
After defeating the creature and going through a door, you will eventually see a red rug on the floor with some candles. Interact with this and examine the sword.
Carry on forward through the next door and interact with the cage at the end for a cutscene.
TROPHY 31/40 - Unrepentant - You sent the Hound away.

RED HOUSE

– Unravel the secrets of Thomas's home –

Go up the stairs into the burnt-down house.
TROPHY 32/40 - Among the Ashes - You found the Red House.
Go over to the broken stairwell on the right and pick up the Framed Family Photo on the floor beside it.
Go into the doorway on the left into the small room and examine the note on the broken furniture.
Troubled Man's Note 9/20 - Demons.
Now go back into the main room and pick up the 2nd Framed Family Photo almost immediately on the left.
Next, go over to the fireplace and pick up the Framed Wedding Photo.
1. Now interact with the left corner of the fireplace and place the Wedding Photo.
2. Next place the round Framed Photo in the middle.
3. Then finally place the remaining Framed Photo on the right.
Now go over to the mirror and enter it.
Go over to the fireplace and pick up the Mask of Joy.
Go over to the table on the right and cut open the large egg-type sac using your razor blade, then pick up the Red Butterfly inside.
Go back through the mirror.
Interact with the doll's house and pick up the Toy Mirror on the bottom floor.
Now put it in the top left room.
Exit the dollhouse and go back through the mirror.
Go over to the bed and interact with the pillow, then take the Mask of Calm.
Now go over to the egg-type sac, cut it open with the razor blade and then pick up the Blue Butterfly.
Then take the Fish Toy from the table on the left.
Go through the mirror back to the material world and then interact with the dollhouse.
Take the Toy Mirror and now place it in the top right room.
Now exit the dollhouse and go back through the mirror.
Go left over to the hanging toy and place the fish toy on it, then pick up the White Butterfly.
Go over to the egg-type sac near the bed, cut it open and take the Yellow Butterfly.
Interact with the end of the bed, then from left to right;
1. Place the red butterfly.
2. Then the yellow butterfly.
3. Followed by the white butterfly.
4. Finally the blue butterfly.
Then take the Mask of Wonder from inside the chest.
Go back through the mirror, head over to the dollhouse, take the Toy Mirror and now place it in the top left room, then go back through the mirror.
Go towards the camera to find a wall of skin, then cut through it with the razor blade.
Interact with the 3 dolls.
1. Place Mask of Joy on the left doll.
2. Mask of Calm on the middle doll.
3. Mask of Wonder on the 3rd doll.
TROPHY 33/40 - The Pact - You uncovered the mystery of the burned down house.
Now quickly go back through the mirror to avoid the moths.
Now go down the stairs where the mirror was that just shattered.
Turn right (left on the camera) and go into the corner ahead, then take the note from the shelf.
Troubled Man's Notes 10/20 - The Missing Piece.
Go into the nearby wine room and take the note from the shelf.
Troubled Man's Notes 11/20 - Found a Way.
Now pick up the shoe on the chair opposite the ledge you dropped down and examine it for an echo.
Echo 26/28.
Head into the room in the corner, then pick up the teddy on the left and examine it for an echo.
Echo 27/28.
Now pick up the syringe on the shelf nearby and examine it for the final echo.
Echo 28/28.
TROPHY 34/40 - Calling Out to Me - Find all Echoes in a single playthrough.
Pick up the note on the cabinet just to the right.
Troubled Man's Notes 12/20 - A Different Kind of Sickness.
Now squeeze through the wall space near the shoe.

– Find a way to open the secret door –

Interact with the door panel and input 1966 to unlock it.

– Investigate the Red House –

Head down the stairs for a brief cutscene.

– Make your way through the underground corridors –

Now sprint down the corridor to escape the creature.

FALLOUT SHELTER

– Make your way through the underground corridors –

Grab the note on the crates opposite the entrance.
Troubled Man's Notes 13/20 - Separation.
Go through the open doorway and cut the wall of skin with the razor blade at the end of the corridor.
Now go through the next doorway on the left.
In this next open room with the catwalk, leave your body.
Now follow the path leading to the top right, then take the first right to enter a room with a spirit well.
Absorb the energy and then return to your body.
Go left and follow the catwalk up the stairs and into another chamber.
Now keep heading straight following the catwalk and it should eventually lead you into a corridor with an electrified floor.
Pull the nearby lever to disable the electrified floor and then head into the control room ahead.

– Pump out the water to clear the way –

Pick up the note on the left side of the control panel.
Troubled Man's Notes 14/20 - A Relic of the Past.
Open the door on the left by pressing the button on the wall beside it, then head on through.
Use a spirit blast on the electrical box to power it.
Go back into the control room and interact with the central panel.
1. Press the right button on water pump 1 so that the central and east chambers are at max pressure, but the rest are empty.
Now go through the doorway on the left and follow the path until you reach the next open chamber with the catwalk.
Go down the stairs on the left and carry on all the way to the end of the path to reach a valve, but make sure you use the spirit shield to avoid the moths.
Turn the valve once you reach it.
Now leave your body and absorb the energy nearby, then go back to your body.
Now head back up to the control room and interact with the control panel.
1. Press the left button on water pump 1 twice.
2. Now press the right button on water tank 1 once.
3. Press the left button on water pump 2 twice.
4. East chamber and water tank 2 should have no pressure now.
Go right through the corridor and into the east chamber.
Once on the catwalk, go down the stairs that you pass.
Keep following the catwalk ahead and go down some more stairs.
Once at the bottom, follow the path around until you reach a locker. Use the bolt cutters on it and get the Valve from inside.
Now backtrack a little to the valve slot you passed on the corner and place the valve on it and then rotate it.
Go back up to the control room and interact with the control panel.
1. Now press the left button on water pump 1 once.
2. Press the right button on water pump 2.
3. Then press the left button on water tank 2.
4. The central and east chambers should now have no pressure.
TROPHY 35/40 - Dark Water - Make it through the pump station.
If you haven't triggered it yet, now unlock the trophy below. You will need to leave your body and re-enter to recover when necessary.
TROPHY 36/40 - Spirit Walking - Spend 10 minutes outside your body.
Now go back all the way back down to where the locker was from earlier.
Now slowly enter the room behind the locker which is now open and use your spirit shield to void the moths as they fly past. Don't run past too fast otherwise a cutscene will trigger which deactivates your spirit shield and annoyingly, the moths will still be nibbling at you!
For this encounter with Maw, you need to interact with the button beside his initial spawn to open the vault door and escape.
Leave your body and sprint through the doorway on the left which is blocked in the material world.
Once Maw follows you in, quickly re-enter your body and he will then be confused for a few seconds, you can then run over to the button and press it to open the vault door.
With the door open, head down into the vault.

THE VAULT

– Investigate the living quarters –

Take the first doorway on your left to enter the power room.
Interact with the power system panel on the left and pull any lever to break it.
Now take out the burnt fuse above the levers on the left.
Go into the lower left corner of this same room and take the note from the central shelf.
Troubled Man's Notes 15/20 - A Thin Line.
Go back to the corridor, turn left and follow the path to the end where you will see a green light beside a child's room.
Go inside the room and take the note from the table on the left.
Troubled Man's Notes 16/20 - Alone.
Now nteract with the panel below the green light and take the Fuse.
Go back to the power room and interact with the power system panel.
Place the new fuse and then flick down the 1st, 2nd and 4th levers to activate the power.
Go into the canteen opposite the power room and take the note from the table on the right.
Troubled Man's Notes 17/20 - Just Me and Her.
Now interact with the broken mirror for a new objective.

– Fix the broken mirror –

Pick up the Mirror Piece on the kitchen counter.
Go back into the corridor, turn right and take the next door on the left to enter the washroom.
Grab the Mirror Piece on the floor.
Go back into the corridor, turn left and enter the next door on the left to enter the living room.
Take the note from the shelf opposite the doorway.
Troubled Man's Notes 18/20 - Keeping Distance.
Also take the note from the desk.
Troubled Man's Notes 19/20 - Business Card.
Take the Mirror Piece from the table beside the TV.
Go to the investigation board on the far wall and take the final note below it.
Troubled Man's Notes 20/20 - Hypothesis.
TROPHY 37/40 - An Unknown Outcome - Find all of Thomas' notes in a single playthrough.
Now go back to the mirror and place all 3 pieces for a cutscene.
TROPHY 38/40 - Mirror, Mirror on the Wall - You completed the broken mirror.

– Make your way to the lake –

Now make your way through the woods to finish the game and obtain the final 2 trophies.
TROPHY 39/40 - Can't Always Save Everyone - You finished the game.
TROPHY 40/40 - Completionist - Collect every other trophy.

First I heard about that, I platinumed it 4 times carefully observing everything and had zero problems except for a few crashes. They did fix some trophies in the last update, maybe it was fixed. A few things have been moved around via updates, but this guide is all up to date!

Amazing guide, yet I'm having several echoes glitches. All of Henry's echos are not popping nor counting on the ps5 system, also the calling out to me trophy for funding them all. I Uninstalled the game, cleared all saves from system and cloud, everything, nothing worked. Guess I'll wait a new patch

Great guide @Optinooby! Only used your text guide but missed 1 echo (or died and missed to recollect an echo) and had to play the game from the start again. The only thing which I didn't liked was that you didn't highlighted the collectibles. Like "Echo 4/28" in a different color. It would make your text guide more readable but it's just a little nitpicking from my side so don't think I want to be rude or something like that. Other than that, thank you for your time it was very helpful and cheers, have a good one!

I understand mate and agree, these types of thread walkthroughs from me though are more just a case of me tidying up my notes so they dont go to waste and pass them on to you guys.

Ideally i'd rather put them into a Gameplay Guide if im going to take the time to present it nicely.

Not myself, but think it has happened to some, you may have to try reinstalling or delete your save altogether and restart.

Are you playing online??

I cant recall if this uses activity cards or not, but if it does, sometimes trophies dont work if you never go online with the game.

Returnal does this, if you never go online at least once while the game is running, no trophies pop iirc

If you've been connected at least once, shouldnt be that.

I just think games that have trophies tied to activity card, like Returnal. If you never go online while in the game, the cards never get loaded or synched up and so no trophies pop. Once those cards have been synched once though, ur good to go even if you disconnect in the future

How many Echos are there in the game?

The guide list 28 but other places show or say that there are 32 echos. Followed the guide and after getting the Syringe, no trophy popped.

Jermster_91, I haven't played Medium, but the PSNP guide by guyblade lists the echos with pictures of each location. Does this help?

Looking over some of the other stuff, the 4 other Echos might be the Phone numbers.

Looks like I will have to finish the story and start a new game to get all the Echos again. I was hoping to get the Platinum tonight but I guess that is not gonna happen. The good news is that I won't have to worry about the Maw detecting me.
