You need a title starting with a Number as well....

For Q only Quest of Dungeons is possible to get

Mh didn't found evidence against this on a quick search. But i had read it wouldn't be achievable here on psnp once. I think even within this topic. Either way i think Quest of Dungeons is the easier one to get

This thread is the only place where I have read about Quantum Theory being unobtainable and I never saw that claim backed up with a link to a server closure announcement. You need 6 people to start an online match and for some of the online trophies you need a full 8 player match. So it's more improbable than impossible as far as I know but I haven't played it online since I got all the online trophies about a year ago. It wasn't a very popular game (understandable) so it's possible, I suppose, the game servers were shut down without bothering to tell anyone and nobody on the whole internet except for in this thread cared enough to notice.

I think the game need a japanese written trophy name and since Clannad is written in latin this won't count...
