Unfortunately, your only likely option then is to delete your profile and re-create it. I don't know if anyone has already posted about the process of this, but here's the general gist:

  1. Back up your save data, screenshots and videos that you want to keep. You can back up your saves to the cloud if you have PS+, or to a USB stick. Screenshots and videos will need to be backed up to a USB stick.
  2. Once you're ready, log out of your account. Select the option to create a new account. Skip logging into PSN. Once you get the main menu, go to SETTINGS > LOGIN SETTINGS > USER MANAGEMENT > DELETE USER. (Make sure you have all your backups done before proceeding). Select your main account and delete it. Log out of the PS4 again.
  3. Select the option to create a new account. This time however, log in with your main account. Once you get the main menu, go to SETTINGS > ACCOUNT MANAGEMENT > RESTORE LICENCES, just to be on the safe side. You might also want to check the ACTIVATE AS YOUR PRIMARY PS4 option in the menu above RESTORE LICENCES, but that shouldn't have been changed by deleting your profile. It's at this point you're pretty much done fixing the issue, and can proceed to restore your saves, etc.

It can take a while depending on how many saves and screenshots/videos you have. You will also lose any folders you've made, and some system settings will return to default, so I suggest going through them all again to make sure they're how you want them. You won't lose any installed games. I can't remember if you lose your installed themes or not, I don't think you do, but you'll need to reset your theme if you had one once you've re-created your profile. You'll likely also be bombarded with notifications for a while about how to do basic stuff, as your console is basically being soft factory reset, but the trophy issue should be fixed at least.

I salute to you, captain, it seems everything is perfect now. You are the best - and ofc everyone else who wrote down their thoughts, everything helped.

This has been happening to me the last month or two. 99% sure the problem started when I just tried streaming Mortal Kombat 11 for 50 PS Stars points - and it's causing constant crashes when starting games as well as skipping trophies (which is obviously considerably more annoying).

Will have to try some of these solutions over the weekend - thanks to all those who posted.

We need a solution from Sony; this issue has been ongoing for at least a decade!

It happened to me for the third time at the weekend. I found a new solution online which seemed to work. The game that caused the error worked ok, and I got its trophies. The I started a new game and oh surprise that error again.

Took me an hour to backup my saves to USB and 3 hours to redo everything else after I deleted my profile. ?

I would think at this stage, the odds on any solution from Sony are around zero. If they haven't bothered in so long, why do so now, with the PS4 approaching it's end of life?

Hey all! I stumbled across this error when I first booted up Pato Box.

At first, I thought nothing of it and tried to boot up the game a second time. I didn't see the error reappear upon rebooting, so I assumed the error must've resolved itself and carried on playing. Trophies unlocked normally up until I wasn't able to unlock a trophy for winning the top prize in a roulette mini-game despite meeting the requirements twice. I closed the game, reopened it, and met the requirements a third time before the trophy eventually popped.

I thought, "No big deal. Maybe it's just a buggy trophy." But, a bit later, I encountered the error again (I had to close and reopen the game) and didn't unlock the :bronze: Gambling Ducks trophy when I was supposed to. Normally (I think), this trophy pops as soon as you collect all five of the casino's Patokens. Instead it popped seconds after beating the casino's boss alongside the :bronze: Lucky 7 trophy.

Again, I thought, "Hmm. Maybe the collectible trophies only pop once you've beaten each area's boss." But, when I earned the next collectible trophy, :bronze: Crocoducks, it instead popped as soon as the boss battle began. At this point, I started to think something was up... I found this thread, backed up the essentials, deleted my profile according to the steps outlined by @Raidou Kuzunoha XIV (thank you!), and the error disappeared.

Now, the reason I'm posting here today isn't because I encountered the error, but because the error may have produced a questionable timestamp.

Specifically, the 9 second gap between my :bronze: Lucky 7 trophy and my :bronze: Gambling Ducks trophy may not be reproducible through normal play considering that you'd have an impossibly brief window to collect any remaining Patokens in the casino after beating the boss (assuming the trophy is supposed to unlock immediately after collecting all five Patokens). However, if any of these collectible trophies can be earned under other circumstances, such as reaching or beating a boss after having obtained the tokens, then it may just be a case of a buggy trophy wherein this sort of timestamp would be reproducible. For the sake of being thorough, I've linked screenshots for Lucky 7 and Gambling Ducks here to show that they both popped after the boss battle.

Is this flaggable? Any help would be greatly appreciated!

Edit: Post revised for clarity.

However, if there are cases of other players unlocking any of the collectible trophies immediately after beating in-game bosses, then it may just be a case of a legitimately buggy trophy wherein this sort of timestamp would be reproducible in rare instances (sadly, I haven't seen any comparable timestamp gaps on the accounts I checked). For the sake of being thorough, I've linked screenshots for Lucky 7 and Gambling Ducks here to show that they both popped after the boss battle.

Questions:
1). Is this flaggable?
2). What should I do?
3). How does the CRT normally handle cases involving the NP-32091-5 error?

Any help would be greatly appreciated!

Pinging the CRT: @MMDE and @Squirlruler.

This is happening to me now, too.

Boot up a game, get kicked out right away, error NP-32091-5 pops up.

Then, I try to sync trophies manually and get error NP-32098-2.

Plus, all the recent PS4 games I played have a bunch of missing trophies; some have none at all.

I tried everything except for deleting my profile which I really don't want to do but I guess I'll do that some day if it's my only option.

This is very worrying and it's ridiculous that PlayStation hasn't done anything to fix this. ?

I'd suggest you to 'fix' the issue sooner rather than later. Back up all saves to cloud/USB, same with all media (photos and videos), then delete your profile and re-add it.

It'll take a while, but it'll fix it going forward. If you don't, it's gonna slowly start refusing to unlock trophies or stop you syncing. Just make sure you've synced your trophies to PSN, if you can - if you can't, any trophy you've not yet synced will be lost when you re-add the profile, but there's no getting around that. But, you're best fixing it so you can sync again rather than continuing and making any more trophy unlocks pointless and technically non-existent on the servers.

Yeah, for sure. The reason why I'm delaying it is because I'm not planning on playing PS4 anytime soon and will get around to it when I have more time. What a pain in the ass this is, right?

Alright - I decided to just get it done and here are some notes for anyone else who may be on the fence:

  • It's a lot less of a pain than I thought it would be and doesn't take that long.
  • You have to delete your profile before you sign in with your account to a new one.
  • Everything seems to work now with no more error messages.
  • The PS4 runs a lot smoother, especially seeing how fast the trophies sync.
  • Some of my settings have been reverted so I had to change them back.
  • I have to re-play a few games if I want to earn the trophies that are missing. While tinkering around a bit with the problematic games, some missing trophies unlocked randomly but I'm still missing a lot of them.

I had this same issue happen to me right when I started playing Sonic Frontiers. I had to delete my profile and recreate it, and things have been running smoothly ever since! I know it's a pain to have to recreate everything, but honestly it's a lot better than having trophies glitch and give you impossible timestamps forcing you to hide your hard earned trophies!

I'm trying to play Jazzpunk today and I receive the message "User will be logged out of the PS4 because an error has occurred", the thing is, I'm receiving 2 different errors, one is this one here NP-32091-5 and the other is NP-32430-2.

I restored my licenses, forced sync throphies with PSN and now I'm going to rebuild database, since the problem still persists

I'm getting only the NP-32430-2, I can sync very slowly. I realized the error occur when trying to install a new trophy sets but sometimes can occur while playing.

If I start the game offline and then sync I can have trophyset installed and start the game.

I really hope that another solution came up because it's too much here to backup.

So it seems the problem is not the new game I'm trying to play but some game before. In my case it was probably Minit or Watchwood.

Update.

So, Like I said I started the game Virginia offline to install the trophies then went online to sync it. It synced.

Then I did a test, I delete a 0% of Legend of Dragoon. Tried to start this game, error.

So after I posted here, I deleted the 0% Virgina from my trophies while connected. An auto synced started( not it takes about a minute, in the past used to be 5 seconds)

I restarted the PS4.

Then I tried to boot Virginia, expected the error, but game booted and put the trophies.

Tried to boot Legend of Dragoon, same thing.

It was fixed? I don't know, I will see what will happen in the next days.

Update 2

I made some mistake after, the Virginia set didn't sync and now I getting the same error as most of you, the 32091-5.

Edit: deleting profile does not delete vids or pics. They reappear when you sign in.

Could anyone corroborate this? I have nearly a terabyte of captures (I am...very bad at tasking myself to crop down videos), and don't want to go through the arduous task of backing all that up if creating a new user profile with the same PSN account would do the trick.

When I had to delete my profile/user on PS4 back in 2019 due to this error, it definitely deleted all my videos and screenshots. Not sure if perhaps a system update since then has changed this.

Yes, all your media is deleted, but you can save it to USB before you delete your profile. Although, you can't  save it back to your new profile, so it seems a bit pointless.

Yeah, found out in my impatience. But, there was no chance I would ever have the temerity to actually cull and curate my Capture Gallery, so recklessly ripping the band-aid off is prolly for the better.

That's bizarre. Was this on PS4 or ps5. On my PS4 still have all my vuds but they aren't listed as created by me. Did you have any other profiles on the playstation?

When I had this issue, it deleted all my own videos and screenshots, and only kept the ones from my brother's account. This was on PS4 btw.

I would say to anyone that has to delete their profile, make sure you upload all saves to the cloud, and backup all the screenshots and videos you like so you don't lose them!

happened to me right now. Can't event delete a 0%. Error all time. If I delete my profile will I lose my download games? I have P.T. Installed.......

I deleted my profile and all games remained, just anything that was specifically mine (trophies, screenshots, saves etc) were gone.  I do have multiple accounts on my PS4, so even after deleting mine I could still see the games albeit with a 'locked' icon.

Your games and DLCs are perfectly safe and will still be there when you restore your profile but backup your saves beforehand, because those will be deleted.

Hi all, I encountered this problem yesterday and thought it was the game I was playing at the time as I wasn't aware of this error. I basically downloaded and installed a couple of games, My name is Mayo 2 and Candy Break Halloween head to head, and had Killing Floor 2 downloading in the background. I got the plat on My name is mayo 2 no problems, sync'd trophies, backed up my save and deleted the game. I then played Candy Break and when I launched the game it came up with a blue screen and something about a problem and being signed out with error code np-32091-5. I logged back in and launched the game again and it was fine, although I noticed some trophies didn't pop when I met the requirements. I closed the game and re-launched it and had the error and repeated the above. Now some trophies popped when I met the requirements again, but not all. So I quit the game and deleted my save and repeated the above. I had to delete my save file twice in order to get the game to pop the remaining trophies. Annoying as a 30min Plat game took about 2 or so hours, and the trophies didn't pop in the correct order. e.g. not getting the trophy for beating the cpu once but getting ones that required a few games played. I sync'd the trophies and deleted Candy Break. I launched Killing Floor 2 and again I got np-32091-5 error code. Now I started getting concerned, as I already had Plat and 98% completion on Killing Floor 2, so I knew the game hadn't caused any issues in the past. I continued to play KF2 and got the latest collectibles trophy bringing me back to 100% on the game, sync'd and deleted. I then spoke to my friend and read up about this error code. As suggested, I launched a few new games to create a 0% trophy list but the problem was they didn't create a new list and only 1 showed up when looking at trophies via the shortcut menu. I then rebuilt the database by choosing option 5 in safe mode and afterwards renewed my licenses from the account management menu. So far I've had no game launch issues and a 0% trophy list was created, it did however throw up a different error code when I tried to delete the trophy list. If the problem persists I will just have to delete my profile, thankfully all saves are in the cloud so shouldn't be a issue. Hope this is of help to anyone.

Edit - Still had the problem so made sure all save files were uploaded to ps cloud and then created new profile and deleted the corrupt one. Signed in on new one. Some games showed a padlock so renewed licenses to fix. Deleting the 0% trophy list now works. So far so good.

my problem seems almost exact to this. i actually noticed maybe a few weeks ago my playstation 4 wouldnt sync when i i manually did it, but all trophies were loading and working fine.

i downloaded a bunch of cheap "jumping" games to kill time for the day. i was having problems with all of them. the trophies wouldnt pop then the game would crash not counting the trophies but saving the progress. i didnt know what was happening. i deleted game and reinstalled but the crashing seems random. i had to delete the saves to go back and re earn the trophies. these are quick 2 minute games luckily not something thats under 1% rare. i thought it was the games themselves and developer since they are cheap games. but once i got done i moved on to lego harry potter. which seems to every other time crash on me on startup and kick me to this code. now im getting worried about this error. when i  try to manually sync, it wont. but my trophies are popping. im just having mainly the startup crash. but i want this fixed. i dont want glitched trophies and time stamps

so from what im reading i guess everyone is saying delete your profile? is that a sure fix?

so i back up all my saves, videos, screenshots, to a usb and then do these steps below and it should fix it? (ps rebuild database didnt work)

also do you have to delete all profiles or just the main one, not sure if its doing it on my alts or if my alts impact my main. although i dont care about my alts

How to Delete a PS4 User Account
  1. Open Settings from the main menu of the console.
  2. In Settings, scroll down and select the option that says Login Settings. ...
  3. Select User Management on the screen that follows.
  4. Choose Delete User.
  5. Select the account you want to get rid of from the list.
